1. Where Do I Get A Birth, Death Or Marriage Certificate?

You can request certified copies of births, deaths, or marriages from Pequannock township’s Vital Statistics and Records Division. Records can be obtained in person or by mail. The event must have occurred within the Township of Pequannock to obtain the record.

Hours of Operation

Monday through Friday (excluding Holidays)
8:30 a.m. to 4:30 p.m.; 

Extended hours to 7:30 p.m. the second Tuesday of each month and 8:30 a.m through 1:00 p.m. the following Friday.

Certificate obtained same day (typically about five minute wait).

• Identification is required (No Exceptions)
  If requesting on behalf of a child, spouse, sibling or parent additional identification is required. You must prove relationship to the individual of record (birth certificate or marriage certificate along with driver’s license or passport)
• $25 fee for first document; $5 for each additional certified copy of the same record. (Cash or Money Order)
• Completed application at Health Department.

2. How To Apply For A Marriage License.

WHERE TO APPLY: Appointment Required Pequannock Township Health Department 530 Newark-Pompton Turnpike, Pompton Plains, NJ 07444 Telephone: 973-835-5700 ext. 127 Office Hours for License Application Process Only: Monday-Friday, 9:00 a.m. - 4:00 p.m.;  (the second Tuesday of the month until 7:00 p.m and the following Friday until 12:30 p.m.) Closed Holiday and Weekends PROCEDURE TO APPLY: You may apply in the municipality where either applicant resides.  If both applicants live out of state you must apply in the New Jersey municipality where the ceremony is to be held. THE PROCESS FOR APPLYING IN PEQUANNOCK TOWNSHIP: Both applicants must present themselves at our office at their scheduled appointment time along with one witness.  You will be required to complete an application, provide proof of residency, and pay a $28.00 application fee (cash or money order, personal checks not accepted.).  Once the application is completed and accepted by our office there is a minimum 72 hour waiting period before the license may be issued.  Once issued the license will be valid for a thirty day period. Anyone 18 years of age or older may apply for a license.  If you are under 18 years of age we request you contact our office for special instructions. 3. How Do I License My Pet?

PET LICENSE APPLICATION FORM 

Dog and cat licenses are available from the Pequannock Township Health Department beginning January 2nd of the current year by completing the mail-in form below. When obtaining your animal’s license you must provide proof that the animal has a current rabies vaccination through October 31st of the licensing year. In addition, veterinarian’s proof of spay or neutering (if applicable) must accompany the license application. Residents may also obtain their dog or cat licenses by applying in person at the Pequannock Health Department, 530 Newark Pompton Turnpike, Pompton Plains, between the hours of 8:30 a.m. to 4:30 p.m. Monday through Friday, and on the second Tuesday of each month until 7:30 p.m. and only 1:00 p.m. the following Friday. Residents who do not license their dogs or cats by January 31st of the current year must pay a late fee of $5.00 and are subject to summons as provided by law.
